To be a leading community–academic primary care clinic that bridges tertiary geriatric expertise with accessible community healthcare, advancing healthy ageing through evidence-based practice, clinical governance, and collaborative care.

Mission

Vision

  1. To function as a community extension of academic geriatric services, working in close collaboration with hospital specialists to ensure seamless continuity of care.

  2. To deliver structured, evidence-based primary and geriatric care, including cognitive assessment, fall risk evaluation, chronic disease optimisation, and functional rehabilitation planning.

  3. To uphold strong clinical governance standards, ensuring documentation, shared care protocols, and outcome monitoring consistent with academic practice.

  4. To support multidisciplinary collaboration, integrating specialists, physiotherapists, occupational therapists, nurses, and caregivers into coordinated care pathways.

  5. To contribute to teaching, training, and knowledge translation, strengthening the link between academic research and real-world community practice.

  6. To empower families and caregivers, equipping them with education, structured care plans, and long-term management strategies for ageing-related conditions.
